Why Good Embryos Still Don’t Implant – Understanding the Hidden Causes
If you’re performing IVF, you’ll spend most of your time worrying about the embryo. Not drinking. Eating right. Trying everything to ensure it’s as healthy and high-quality as possible. You get top marks from your embryologist.
And then they don’t implant.
That’s a sad truth pregnancy specialists don’t always talk about. Even “good embryos” don’t always implant. In fact, in IVF, only 25-30% of embryo transfers result in successful implantation. That’s why multiple embryos are used to increase the success rate of a single treatment.
But is there anything you can do to prevent embryo implantation failure? And is there anything behind recurrent implantation failure? Find out below.
What is Recurrent Implantation Failure?
Recurrent implantation failure (RIF) is the repeated inability of an embryo to successfully implant in the uterus after multiple IVF cycles, especially if the embryos were of high quality. It affects approximately 10% of couples undergoing IVF cycles.
That means you’re not alone — nor does it mean pregnancy won’t occur.
While it’s important to understand the causes of RIF, the emotional struggle isn’t often discussed. Couples often put all their hopes on treatment. It’s disappointing when a single IVF cycle fails. It’s exhausting when you’ve had three unsuccessful cycles.
But even a third failure isn’t the end. It just requires you to reconsider what could be going wrong and work to fix the underlying problem.
5 Reasons Why Embryos Don’t Implant – Chinese Medicine Perspective
Implantation success depends on the uterus, or specifically, the uterine lining (the endometrium). If this lining is thin, the embryo struggles to implant or survive. A thin endometrium is associated with poor blood flow, inflammation, and immune dysfunction.
The aim : is a warm, nourished uterus with a thick, receptive lining. That depends on smooth qi flow and balanced Kidney and Spleen energy.
1. Kidney Essence Deficiency
Kidney Essence is crucial for growth, development, and reproduction. Indeed, this essence is often used during the reproductive process, fueling normal sexual function. Weak reproductive essence contributes to a thin or undernourished endometrium.
Symptoms you may experience : Fatigue, low back or knee weakness, irregular periods, poor ovarian reserve.
2. Spleen Qi Deficiency
Spleen Qi Deficiency is a common cause of disharmony caused by an unhealthy diet, emotional stress, or mental exertion. In TCM, the Spleen transforms food and drink into Qi and Blood, which then nourish the uterus, ovaries, and endometrium. If the Spleen Qi is weak, it leads to poor blood flow to the uterus, reducing uterine nourishment. Spleen Qi Deficiency can make an embryo‑friendly lining hard to build and hard to maintain, even when the embryos themselves look “perfect.”
Symptoms you may experience : Poor appetite, bloating, loose stools, pale complexion, easy fatigue.
3. Liver Qi Stagnation
In TCM, the Liver ensures the smooth flow of Qi and stores and regulates Blood, including the blood that nourishes the uterus and endometrium. When Liver Qi does not flow, heat accumulates in the Liver. Closely linked to emotional distress, it disrupts blood flow to the uterus. Women may notice painful or irregular periods, feel moody or irritable, and experience bloating in the stomach.
Symptoms you may experience : Irritability, mood swings, PMS, abdominal tension, irregular cycles.
4. Blood Deficiency
Blood is one of the most vital substances in the body, if not the most vital substance. Blood flow supplies nutrients to the uterus, supporting the endometrium as it thickens and grows. Insufficient Blood — caused by blood loss, poor diet, physical and mental stress, or chronic disease — leads to scant or absent periods and a thin, undernourished uterine lining.
Symptoms you may experience : Pale or dry complexion, dizziness, scanty periods, brittle nails, insomnia.
5. Cold or Damp Obstruction
Cold or Damp Obstruction describes a uterus that is physically present but energetically cold. Often known as “cold uterus” so even good embryos struggle to attach and grow.
Excess cold or dampness in the lower abdomen creates a hostile environment for successful implantation, it slows circulation and slows circulation and tightens vessels, this can create problems with implantation or early pregnancy.
It’s the opposite of that warm, nourished environment that supports a healthy embryo. In TCM, the uterus needs a gently warm, well‑circulated environment so blood and Qi can nourish the endometrium and hold an embryo in place. Dampness and cold are examples of the Full Cold pattern, which often occurs in women who eat excess cold foods, live in a cold environment, or spend long periods outside during winter.
Symptoms you may experience : Lower abdominal cold, edema, heavy vaginal discharge, sluggish digestion.

Treatment to Improve Embryo Implantation
1. Acupuncture Can Improve Uterine Blood Flow
Acupuncture is the ancient art of inserting fine needles into specific parts of the body to counteract imbalances. A growing body of evidence supports its effectiveness.
One potential benefit is increasing blood flow to the uterus.
In one RCT (randomized controlled trial), 69 women underwent either traditional or sham acupuncture. They found significantly improved uterine artery blood flow in women undergoing IVF compared to the control group. Their conclusion? “Acupuncture might be beneficial in women undergoing IVF-ET by increasing endometrial blood flow and endometrial receptivity.”
These findings are consistent with other clinical studies. A 2009 study discovered that electroacupuncture increases blood flow in uterine arteries. Similarly, a 27-study systematic review (including 6,116 patients) found that acupuncture resulted in a significantly higher pregnancy rate. That was most notable among women with a prior history of unsuccessful IVF attempts, which may have been linked to recurrent implantation failure.
Finally, a 2020 study in 72 patients saw an increase in endometrial thickness after acupuncture. These women had struggled with implantation failure despite high-quality embryos. The pregnancy rate was also significantly higher: 47% to 33%.
2. Personalised, Cycle-Specific Treatment
Acupuncture isn’t the only treatment. Other treatments focus on the menstrual cycle phases — follicular, ovulatory, or luteal — and the underlying problems.
Each treatment depends on the deficiency or stagnation pattern. For example, Cold or Dampness Stagnation can be soothed with warm, nourishing foods and reduced cold exposure. Liver Qi Stagnation requires reduced stress, a focus on Blood-generating foods, and even specific herbs.
When the treatment is tailored to the menstrual cycle phase, underlying pattern, and the IVF schedule, it may improve uterine receptivity and implantation success. It’s about creating the optimal conditions for implantation.
