Large Fibroids : Does Size Really Matter For Fertility Treatment?

Fibroids, are an abnormal growth within or on the uterus. While they’re benign and non-cancerous, fibroids can cause problems during periods, conception, or pregnancy especially when they’re bigger in size.

Most women with fibroids won’t have any symptoms. It’s estimated that around 1 in 3 women will develop fibroids at some point during their life and experience symptoms. Many women with fibroids can still get pregnant naturally, but in some cases fibroids can affect fertility. The size of the fibroid can make a difference in the result.

What is Considered a Large Fibroid?

Most fibroids are not considered large. Only when there are persistently higher levels of female hormones will the fibroid continue to grow.

Fibroids are categorised as follows:

  • Small fibroids — between 1 cm and 5cm
  • Medium fibroids — between 5cm and 10cm
  • Large fibroids — anything bigger than 10cm

A single fibroid greater than 10cm is considered large. But fibroids can grow even bigger,  up to a grapefruit or larger. If it gets too big, it can distort the inside and grow outside the uterus. In the most extreme cases, fibroids can fill the pelvis or stomach, giving a false appearance of pregnancy. However, this is very rare.

Symptoms of Large Fibroids

Most women who have uterine fibroids will never develop any symptoms.  Symptoms depend on the size and number of fibroids and their location. For example, a fibroid’s location could block the fallopian tubes or sit over the cervix, causing fertility issues, or it could press backwards, affecting your bowels. Symptoms include:

  • Heavy or prolonged menstrual bleeding
  • Pelvic pressure or a feeling of fullness
  • Lower abdominal or pelvic pain
  • Visible abdominal enlargement or bloating
  • Frequent urination or difficulty fully emptying the bladder
  • Constipation or rectal pressure
  • Pain during sex
  • Lower back or leg pain
  • Anaemia-related symptoms (fatigue, weakness, breathlessness)
  • Fertility or pregnancy complications (depending on size and location)

Complications Due to Large Uterine Fibroids That Can Affect Fertility

  • Pregnancy and labour complications, if the cervix is covered.
  • Repeated miscarriages due to implantation issues.
  • Fertility problems (much rarer)
  • Urinary tract infections may occur if the fibroid presses against the bladder.
  • Iron-deficiency anaemia from heavy bleeding.

Part of the reason for these complications is a disrupted uterine environment. In Traditional Chinese Medicine (TCM), a cold uterus is a primary cause: low blood flow, low uterine thickness, and low success of fertility. In contrast, a warm uterus is essential, providing a steady supply of blood and energy to both the uterine lining and growing foetus.

What is the Largest Fibroid Size TCM Can Help With?

TCM is suitable for most fibroids. However, if a fibroid reaches a certain size, lifestyle changes, TCM treatments, and herbal remedies are unlikely to cause significant changes. Even if the fibroid shrinks, it will still remain large enough to lead to disruption.

TCM works best for:

  • 5–6cm fibroids — it is possible to shrink the fibroid meaningfully over time.
  • 6–8 fibroids — improvements in symptoms and sometimes reduced size. Complete resolution is not always possible. Realistic, fertility-focused goals.

When TCM is Most Effective for Large Fibroids

Under TCM principles, fertility is dependent on a uterus that is well-supplied with Blood. It must be warm and receptive, unobstructed by stasis. Large fibroids disrupt this by blocking Blood flow to the uterine lining and  creating an environment less supportive to implantation.

But that’s not the end. TCM can work by improving endometrial quality, supporting ovulation and luteal phases, and strengthening pelvic circulation. Together, they increase the implantation potential and create an environment with the best chance of a successful pregnancy.

Stabilising or Modestly Shrinking Fibroids

Fibroids rely on blood flow and abnormal hormonal levels to grow. Fibroids occur due to a combination of:

  • Blood Stasis (hard, fixed masses)
  • Phlegm/Dampness (thickened tissue)
  • Long-standing imbalance

In this case, Chinese Medicine treatment focuses on moving Blood, transforming Phlegm, and regulating Qi to soften masses, ease congestion, and improve bleeding, with realistic goals of symptom relief and small volume changes rather than complete disappearance of large fibroids. Also, by stabilising hormones, TCM minimises the biological signals promoting growth. That being said, a fibroid that has been present for years can become more “fixed” and resistant — less responsive to shrinkage.

Preventing Further Growth

Chinese Medicine is not only great at managing existing Fibroids it is also great at preventing future fibroid progression by targeting the underlying imbalances that allowed them to develop initially. This is achieved by:

  • Regulating Liver Qi to to reduce stress-related hormonal fluctuations
  • Supporting Spleen function to improve fluid metabolism and prevent tissue thickening
  • Strengthening Kidneys to support long-term reproductive and hormonal health

Even if the Fibroids are very large to be shrunk, their size can be stabilised and prevented from growing further, this is especially useful if you are undergoing IVF.  To achieve this treatment goal, treatment looks like on going herbal treatment plan, acupuncture as well as diet and lifestyle changes.

How TCM Treatment Changes When IVF is Involved

IVF changes the equation slightly. Instead of aggressively trying to shrink the fibroid, the goal shifts to creating the right environment for a successful pregnancy. This includes supporting egg quality, uterine receptivity, and implantation.

Our experts typically recommend starting Chinese medicine treatment 3 months prior to IVF. Treatment is broken into several stages:

Preparation before IVF – As during this stage there is no risk to the embryo, Acupuncture and Chinese medicine herbs are used to regulate menstrual cycle, support ovaries and actively work on reducing stasis related to fibroids.

During ovarian stimulation – During this process it is recommended to maintain acupuncture to 1 -2 times per week, this helps with improved blood flow, better management of stress and assist with any side effects.

The egg retrieval process, and after transfer – Here the goal with the Chinese medicine treatments is to clam the uterus, tonifying Kidney and Spleen, and nourishing Blood. All of which can contribute towards increased IVF success rate.

Treatment for Large Fibroids

1. Acupuncture

Acupuncture is a mainstay in Chinese Medicine, acupuncture for fibroids works with the goal of improving uterine environment rather than shrinkage fibroids. It takes an holistic approach, by improving blood circulation, balancing hormones and eliminating stagnation in the pelvic region. Studies have shown that acupuncture works by increasing blood flow to the uterus which can influence fibroid growth and relieve symptoms such as heavy bleeding, pelvic pressure, pain, and fatigue, while also supporting fertility by improving endometrial quality and uterine receptivity.

According to the latest research, Acupuncture is highly effective at addressing uterine fibroids (alongside Chinese herbs). One review of nine studies found that acupuncture combined with Chinese herbs “reduces uterine size, lowers hormone levels, and improves quality of life.”Another study confirmed the finding. It tested acupuncture on 8 participants with uterine fibroids.  All 8 of the subjects saw improvements in symptoms within 6 weeks of treatment.

2. Supplements for Large Fibroids

Supplements can support the management of fibroids by helping to balance hormones, reduce inflammation, and improve uterine circulation. Used correctly, they may help stabilise fibroid growth, ease symptoms, and support overall reproductive health. Below are our most recommended supplements; however, we strongly advise speaking with a qualified practitioner, as a personalised treatment plan tailored to your specific fibroid type, size, and fertility goals is far more effective.

FAQ

What size fibroids need surgery?

Generally, if a fibroid is over 5–6cm, surgery may be recommended, depending on symptoms, location, and fertility goals.

Is a 7cm fibroid dangerous?

Yes, any fibroid over 5cm is potentially dangerous. It requires medical attention and possibly surgery.

Do fibroids always require surgery?

Not always. Surgery is considered if fibroids cause severe symptoms or block fertility, but many women benefit from TCM, lifestyle changes, supplements, and acupuncture to manage fibroids non-invasively.

Can fibroids cause pain or heavy bleeding?

Yes, especially during menstruation or ovulation. TCM treatments aim to improve blood flow, reduce stagnation, and relieve pain naturally.

Can fibroids affect my ability to get pregnant?

Yes. Large fibroids can distort the uterine cavity, block fallopian tubes, or interfere with implantation, making conception more difficult

Can TCM help with fertility if I have fibroids?

Yes. TCM can improve uterine blood flow, support endometrial health, regulate hormones, and reduce inflammation, creating a more receptive environment for implantation, even if fibroids cannot be completely removed.

Should I have fibroids removed before trying to conceive?

It depends on the size, location, and type of fibroid, as well as your fertility goals. Your specialist may recommend surgery for large submucosal or cavity-distorting fibroids, while TCM can be used before and after surgery, or as a non-invasive support.
